اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ عَذَابِ الْقَبْرِ وَمِنْ عَذَابِ جَهَنَّمَ وَمِنْ فِتْنَةِ الْمَحْيَا وَالْمَمَاتِ وَمِنْ شَرِّ فِتْنَةِ الْمَسِيحِ الدَّجَّالِ
Allahumma inni a'udhu bika min 'adhabil-qabri, wa min 'adhabi jahannam, wa min fitnatil-mahya wal-mamat, wa min sharri fitnatil-masihid-dajjal
O Allah, I seek refuge in You from the punishment of the grave, from the torment of the Fire, from the trials of life and death, and from the evil of the trial of the False Messiah.
The Prophet ﷺ would seek refuge from these four things so regularly that Abu Hurayrah (RA) said: "When one of you has finished the last tashahhud in prayer, let him seek refuge with Allah from four things." This du'a covers the major fears of the unseen: the grave, Hellfire, the trials of life, and the greatest trial before the Day of Judgment.

اللَّهُمَّ إِنِّي أَسْأَلُكَ الْجَنَّةَ وَأَعُوذُ بِكَ مِنَ النَّارِ
O Allah, I ask You for Paradise and seek refuge from the Fire.
Sahihإِنَّ لِلَّهِ مَا أَخَذَ وَلَهُ مَا أَعْطَى وَكُلُّ شَيْءٍ عِنْدَهُ بِأَجَلٍ مُسَمًّى
To Allah belongs what He took, and to Him belongs what He gave, and everything with Him has an appointed time.
Sahihاللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ فِتْنَةِ الْقَبْرِ وَمِنْ عَذَابِ الْقَبْرِ
O Allah, I seek refuge in You from the trial of the grave and from the punishment of the grave.
Sahihلَا إِلَهَ إِلَّا اللَّهُ
There is no deity except Allah. (Encourage the dying person to say these as their last words.)
