اللَّهُمَّ اجْعَلْ قَبْرِي رَوْضَةً مِنْ رِيَاضِ الْجَنَّةِ وَلَا تَجْعَلْهُ حُفْرَةً مِنْ حُفَرِ النَّارِ
Allahummaj'al qabri rawdatan min riyadil-Jannah wa la taj'alhu hufratan min hufarin-Nar
O Allah, make my grave a garden from the gardens of Paradise, and do not make it a pit from the pits of the Fire.
The Prophet ﷺ said: "The grave is either a garden from Paradise or a pit from the Fire." Two possible states for the same 6x2 foot space. The du'a is stark in its binary: garden or pit, no middle ground. What determines the outcome is your deeds and your faith. This du'a asks Allah to convert that dark, narrow space into an expansive, illuminated meadow of Paradise.

O Allah, I ask You for a good ending and I seek refuge in You from a bad ending. O Allah, make the best of our deeds their conclusions and the best of our days the day we meet You.
Hasanاللَّهُمَّ اغْفِرْ لِحَيِّنَا وَمَيِّتِنَا وَصَغِيرِنَا وَكَبِيرِنَا وَذَكَرِنَا وَأُنْثَانَا وَشَاهِدِنَا وَغَائِبِنَا
O Allah, forgive our living and our dead, our young and our old, our males and our females, our present and our absent.
Sahihاللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ فِتْنَةِ الْقَبْرِ وَمِنْ عَذَابِ الْقَبْرِ
O Allah, I seek refuge in You from the trial of the grave and from the punishment of the grave.
Sahihلَا إِلَهَ إِلَّا اللَّهُ
There is no deity except Allah. (Encourage the dying person to say these as their last words.)
